Important and relevant information from the study will be shared with you through updates at the Canine Valley Fever Project (CVFP) website. Results from the Study will be published in scientific journals. Dogs that are positive for Valley Fever will constitute a registry, which will provide valuable cases studies for future veterinarian study. Right to Withdraw After the Screening Visit, if you do not wish to participate simply let your Study Vet know. If you do decide to continue with the Study and have to withdraw later on that is also acceptable. Please just notify the Study Vet. Rescued and fostered dogs that are then adopted may continue in the Study or stop. The identifying number will stay the same in the system. Your Study Vet will make a note of the change in the clinic records. New pet owners that wish to continue need to follow the terms of this website to purchase their labs. They will have to keep the dog at the same Site and under the same identifying code. Copyright Protection and Participation in Other Research The Study software has been especially designed to provide a wealth of data regarding dogs with Valley Fever. The content is copyright protected and cannot be printed and shared with other research institutes. Once the Study concludes, the data will be summarized and shared with the research communities as publications in scientific journals. If your dog is currently enrolled in the Morris Animal Lifetime Study of Golden Retrievers may also enroll in the CVFP. Dogs may not participate in any other studies while they are actively enrolled in the CVFP minimum year term or longer. PLEASE NOTE - lab work/information from CVFP cannot be shared with other studies specifically those conducted by the Valley Fever Center of Excellence or UC Davis Veterinary Medicine. These entities need to provide their own option for lab work and data collection for pets. Owner Consent to Participate I am the legal owner of the dog and am 18-years or older. For rescue dogs, I am the custodian. I have read all of the information provided. I have the authority to execute this release and I consent to voluntarily participate in the Study by purchasing the required labs. I consent to participating in the Study by providing access to information and biological samples related to my dog’s health, lifestyle, diet, and environment and any other requested information. Information and Sample Submission Agreement I hereby donate, assign and transfer all information collected regarding biologic samples to the CVFP. I understand that any future use of the data from these samples will be with the sole direction and authority of the CVFP. I hereby relinquish all rights to, and ownership of, the information and data that are obtained from the dog that participates in the Study. I will not claim any right to ownership of any information or intellectual property rights arising from the Study, and I acknowledge that neither I nor my heirs will gain financially from any discoveries, whether or not of a commercial nature made using the information or samples provided. Owner Consent to Release Information I give my permission for my veterinarian to release information from my dog’s medical record to the CVFP. I give CVFP permission to release information from my dog’s medical record as long as I am not identified by last name. By purchasing the labs I am providing informed consent for participation in the CVFP. |
