Canine Valley Fever Project - How to use a Study Site or your own vet
Pet Owners, please read this section below and follow the steps. Please read everything!
It is recommended to use regular browsers to complete documents. When completing the documents do not leave any red items.
If there is a study site within a reasonable distance, please use the site. We will be adding more sites in the future. Scroll down the page and you will see the Official Study Sites. These vets have all of the supplies on hand and are experienced with the study and Valley Fever in dogs.
IMPORTANT: The study is closed on weekends and holidays. You must allow 24-48 hours on business days for processing paperwork AFTER you purchase the labs and complete the registration.
IF YOU WOULD LIKE TO USE A STUDY SITE OR YOUR OWN VETERINARIAN FOLLOW THESE STEPS:
Step 1: If you are not using a study site then call and schedule with your vet clinic and let them know you are having a blood draw that will be shipped as part of a Valley Fever Study. Please ensure that the clinic approves this and explain that you will be purchasing the lab work from the study at a discounted rate that is separate of the clinic. This does not include any of their other charges such as exam, blood draw, X-rays etc. Forms and instructions will be emailed.
Step 2: Register at Pet Owner Registration under Pet Owner Info and let us know the day and time of the appointment. Only register once. We recommend scheduling Monday through Wednesday ONLY. Make sure you get a confirmation that your registration was received. Please read the Participation Agreement click on agree and it takes you to the lab purchase.
Step 3: Purchase the entry labs which is Test Code 933 from the Purchase Tests tabs ($125). DO NOT purchase titer only labs ($65) for entry. Please allow 1 - 2 full business days for us to process the paperwork. Please use the same last name to purchase as your pet is registered under.
IMPORTANT: We must issue a Study ID to all of the clinics, so the above information is required to do this. Nothing is shipped to you or the clinic.
Step 4: If you are not using a Study Site, we will send to you and the vet clinic through email 3 items: Lab Submission form, Prepaid shipping FEDEX Stamp, and instructions for the draw and shipment. Study Sites have all of the paperwork on hand. but will need a Study ID.
Step 5: Take your dog in for the blood draw. Make sure they are fasting 6-9 hours prior. A full physical exam is up to your vet based upon the health of your dog and new patient status. They will charge at least a vet tech blood draw fee and any other diagnostics that might be necessary for proper medical care.
Step 7: For results it takes at least 3 to 8 business days due to the Cocci Titer.
Step 8: For each follow-up visit make sure you complete the Owner Questionnaire. Do this by clicking on the Pet Owner Info tab and in the drop-down menu you will see the Owner Q. Do this for each visit including the first one. It is required to be in the study.
Step 9: For follow-up labs check with your vet and see if you need the full battery (TC 933) or just the Cocci Screen and Titer (TC 706). Titer only is if you are going to the vet regularly and not for annual visits. You will need to purchase the test prior to the visit and email us the date and time of the visit.
